I'm wondering why a Chaplain/Spiritual Counselor was not also involved as it sounds as if wife may be having some spiritual struggles.

Was any cognitive assessment done to show that he was in fact capable of making his own decisions?

Did the spouse really have the ability to revoke hospice services? It sounds that, even though there was a POA in place, that the patient still had capacity.

Would this be a vulnerable adult situation?

Why did hospice honor the POA when he is alert and oriented?

Agree with ^^ Nikki - did the POA suggest that she could make those decisions even with having capacity

Do you have palliative care program?

Does the State in which the patient resides require Decisional Capacity???
